Two kinds of glimepiride administration: a single dose of the immediate-release formulation and multiple doses of the extended-release formulation (ER glimepiride) were compared in 23 Japanese type 2 diabetes mellitus patients. ER glimepiride was administered once daily to each patient, and the patients continued the administration of ER glimepiride for 16 weeks. Blood samples were obtained before and 4 weeks after the start of the administration of the respective formulations. There were significant reductions in plasma glucose levels, fasting serum insulin levels and homeostasis model assessment-insulin resistance index (HOMA-R) in the immediate-release formulation group but not in the ER glimepiride group. Plasma insulin levels and HOMA-R also decreased significantly in the ER glimepiride group. Fasting serum C-peptide levels and C-peptide response to the glucose challenge test showed no significant differences between the groups. Although the fasting plasma glucose (FPG) levels at baseline were significantly higher in the ER glimepiride group, no significant differences in the HbA1c levels between the two groups were observed. The mean (SD) glucose concentration at 2 h after a glucose tolerance test (GTT) and that at 4 h after a meal tolerance test (MTT) were significantly lower in the ER glimepiride group. In conclusion, ER glimepiride showed a favorable therapeutic effect on type 2 diabetes mellitus, including a decrease in HOMA-R, in contrast to the single-dose (immediate-release) administration of the same drug.Taenia crassiceps
